Congratulations on being nominated for a College of Engineering Alumni Award! There are many things to prepare for the banquet where you will be honored, and we need your help. Please complete this form in its entirety no later than February 1*. If you need to pause and return to the form, your answers should be saved. Please note: If you are an early career or distinguished awardee you will NOT need to prepare any comments for the banquet, but if you are being named as a Hall of Fame awardee you WILL need to prepare up to FIVE minutes of comments. Once you've submitted the form, you will receive a completion receipt that includes your responses and a link to the RSVP form.
*If you do not complete the form on time we will be forced to choose how your name appears on our printed banquet invitation.
