This is a general application form for the University of Arkansas undergraduate students majoring in the MATH or STAT program.

Prepare Required Information Before Hand
FORM MAY TIME OUT AFTER 10 MINUTES OF INACTIVITY (FOR DATA PROTECTION)
The Department of Mathematical Sciences requires students to submit this scholarship application with the following information on hand.
- Any Addresses used by the applicant (local, permanent if any)
- High School Information (including its address and your GPA, Rank, and Date of Graduation), or Education Prior to College
- University of Arkansas Student ID Number
- Recent mathematics instructor's notice of support or letter of recommendation (you can send email requests within the form!)
- Information pertaining to monetary awards that have been awarded less than one year ago or are still actively paying out
Accepting Your Award
You will be contacted if you are selected to receive a Department of Mathematical Sciences Scholarship. Due to COVID-19 precautions, the requirement that students accepting a Department of Mathematical Sciences attend a department gathering & presentation may be temporarily halted. Other arrangements will be made and communicated directly to those students if necessary.
