Form Information

This form covers four types of requests:

Estimate requests.  A member of our Work Induction Estimating and Scheduling (WIES) team will contact you to set up an onsite visit to discuss the project. They will provide either a preliminary project cost assessment (PPCA) or a detailed construction cost estimate (DCCE).

Space requests include expanding or acquiring new spaces, moving offices, and changing the use of a space.   

Small renovation requests include modifications to existing spaces, repainting, carpet replacement, installing new fixtures or equipment, rearranging or ordering new furniture.    

Fixed-Fee Services are a subset of small renovation projects that can be initiated without the estimation process, prices are posted on the FAMA Rates and Fees Schedule.

STEPS TO START A PROJECT

  1. Fill out the Form > Review Submission > Confirm Submission > Receive Confirmation Email
  2. Facilities Management Review > Project Scoping (Contact and/or Site Visit) > Project Plan
  3. Project Estimating > Receive Estimate by Email > Respond to email and Provide the Work Tag.
  4. Work Tag Approval > Work Order
